Among the challenges is defining "failure" in either a product or process.

Measurement of First Pass Yield (FPY) has become commonplace.  However, many organizations fail to publish a comprehensive definition of the term.  LCS developed the following flowchart to assist trainers with the task of educating employees.

One of the tools that we use regularly is a correlation matrix to help identify process measurements that are statistically reliable predictors of higher-level measurements.
